This page displays patient right information including information about accessing your health records and filing complaints as required by Texas Law HB 4224.

Your Health Records & Filing Complaints

You have a right to request a copy your health records from The Dragonfly Harbor. To request your records, please follow the steps below:

1. Submit a written records request. You can do this by letter or email. For email, please note in the subject line, "Heath Records Request" so that we can note your message quickly. We also have Release of Information forms available that can be used to request records or have records sent to other facilities.

2. All Records Requests need to contain the following information:

Your full legal name (and dependents full legal name, if applicable), Date of birth, Date range of records requested, Specific records needed, Method of delivery requested (email or fax), contact phone number.

3. Identity verification is required for records requests. We may need to verify or have proof of your identity for your protection before continuing with request.

4. Expected Response Time. We will respond to your request within 15 business days as required by Texas Law.

*Please note: Reasonable fees may apply for copies of records. We will inform you of any fees in our response to your request.

How to File a Complaint

We always want to know if there are issues with your experience or services at The Dragonfly Harbor. Please contact us so that we may resolve any issues. You may reach out to our email at admin@thedragonflyharbor.org or call our number at 318-232-2232. We will do our best to resolve any issue promptly. We will take your concern seriously as the quality of your care matters to us!
